INDIANAPOLIS - I-Team 8 has learned details about the Indiana Department of Labor's investigation into this weekend's deadly construction accident. One man died and another was injured when they fell from scaffolding at the site where a new movie theater is being built at Castleton Mall.
The police report says a pin was not properly positioned to hold a straight bolt in place on the suspended scaffolding.
Monday afternoon the deputy commissioner for Indiana Occupational Safety and Health Administration told I-Team 8 that investigators determined only three lifelines were on the system even though four people were on the scaffold. They also determined one of the cables apparently broke on the two-point safety harness.
Investigators also are looking into whether the scaffolding was properly set up.
